I'd like to see if I can get this list up to one thousand. I'm sure that I've missed a lot of obvious ones, and even more obscure ones.

The one-shot Star Studded Comics has two possibilities, Commandette and Ghost Woman. Currently p1 of the scan is the start of the Ghost Woman story. The start of the issue is at p17.
Commandette has a good claim. This was her only appearance, and in-story her code-name and costume are from a movie she's starring in. But the item has a story title separate from the "Commandette" logo, indicating she was intended as a possible ongoing character.
Ghost Woman is iffy-er. On the cover she looks like a female Spectre. But in-story she says "Now I can rest! at the end, as if no sequel were intended. The item has a "Ghost Woman" logo, but it can be taken as the story title.
Interesting. Apparently, they brought her back years later as Strong-Woman.
